Students who have completed all graduation requirements are required to prepare and submit the following documents to the Student Affairs Office for graduation procedures:

1. Student ID Card
2. Graduate Student Petition (Click here)
3. Diploma Request Form (Click here)
   (Please complete the Diploma Request Form if you would like your diploma to be sent to your address by courier.)
4. No Outstanding Library Debt
   (Graduation procedures will not be completed for students with outstanding library debts.)
